How to Select Forms Directly from the Mobile Forms App

The Select Forms feature on the forms app enables providers to choose which forms to populate on the app without needing to assign or push them from the web platform. 

How to select forms

  1. Tap on the “Select Forms” button on the top-right of the home screen.


  1. The next screen will show you a list of active forms you can choose from

    1. To learn how to organize your forms into different categories, click here 


  1. Next, select which forms you want the patient to fill out

  2. If the form has a yellow exclamation mark next to it, the provider is required to fill out that field before handing the tablet over to the patient.

  3. To go to the next screen, select “Next” 


Select a Patient

After selecting the forms, you will be taken to the “Select a Patient” screen. This step is optional and allows you to associate the selected forms with an existing patient profile..

By associating a patient:

  • Adit can pre-populate the patient’s information (like First Name, Last Name, etc.) into the form.

  • Once completed, the form can be automatically imported into the patient's profile in the EHR. 


If no patient is selected, the form will be treated like a public form link, and the submission will need to be manually approved and matched to a patient before it can be imported.

How to Associate a Patient to Forms:

  1. Tap the “Select Patient” dropdown.


  1. You can search a patient by their name or phone number


  1. Once the patient is selected, tap on "Next".

  2. if you would like to skip selecting a patient tap on "Skip".


Pre-Fill Fields

The Pre-Fill Fields screen displays all forms that require a pre-fill.. These fields must be completed before handing the tablet to the patient.
When selecting forms on step one, the forms that required pre-fill fields are marked with an orange warning icon (!).

  1. Review and fill out each pre-fill field required




  1. Once all required fields are filled out for a form:

  • The form is marked with a green check mark to confirm completion.

  • The section will collapse, and you can continue to the next form in the list.


  1. You can expand a form clicking the plus icon (+) or collapse a form using the minus icon (-)

  2. Once the fields are filled out, select “Next” 

Submitting a Patient Form

  1. Once the patient forms populate on the screen, hand over the tablet to the patient to begin filling out. 

  2. When the patient is finished, they will press the submit button.
