How to send forms to your iOS or Android device using Adit Patient Forms

The Adit Patient Forms app allows you to send forms to your office's iPad, tablet, or mobile device in real time. This ensures that patients who were not able to fill out their digital forms before their appointment can do so once the arrive at the office.

To use this feature, you must first connect a tablet or other device device to your Adit account using the Adit Patient Forms app: How to Connect your iOS or Android Device with Adit's Patient Forms

Once you have the Adit Patient Forms app set up, you can easily send a patient's forms to your office mobile device.

  1. There are several ways to send a patient form to your office device. Choose one of the following:
    1. Quick Text
      Click the Quick Text button and select the Patient Forms icon. Search for and select your patient, then choose the Push to Device option and click Next.
    2. Patient Card
      From the Patient Card, locate the Patient Forms section. Click the Form Request button.
    3. Form Reminder
      Navigate to the Patient Forms module.
      Under the Requested tab, locate the specific form and scroll to the right, then click the Send Reminder icon.
       
    4. Submitted Form
      Navigate to the Patient Forms module.
      Under the Submitted tab, locate the specific form and click on the patient's name to open it.
      You can click Resend Submission or Send for 2nd Sign; both options will allow the form to be pushed to the device.
  2. The next screen will ask which forms you would like to push to the device. You can select up to 5 forms, and they can be any combination of forms that were previously assigned to the patient or new forms that you are assigning for the first time. When you have selected your forms, click Next.
  3. Select which device you would like to push the forms to, then click Send.
  4. You will see a confirmation message, and the selected forms will load on the iPad, tablet, or mobile device. You can then hand the device to the patient to complete their form(s).

Please note: For patients' security and privacy, any forms pushed to a device will time out after 30 minutes (or when submitted).

Once the forms are successfully submitted, you will be notified in your Adit platform as normal.
