How to Connect your iOS or Android Device with Adit's Patient Forms

The Adit Patient Forms app allows you to send forms to your office's iPad, tablet, or mobile device in real time. This ensures that patients who were not able to fill out their digital forms before their appointment can do so once the arrive at the office.

To use the Adit Patient Forms app, you will need a tablet or mobile device for your office. This device must use either an iOS or Android operating system (ChromeOS and Fire are not supported). If you are unsure if your device is supported, see if it has the Apple App Store (iOS) or Google Play store (Android). If it does not have either of these, it is not supported.

Follow the steps below to set up the Adit Patient Forms app on your office tablet or device:
  1. Go to the Google Play Store (on an Android device) or the App Store (on an iOS device).

  2. Search for "Adit Patient Forms".  The icon for the correct app will look like this:
  3. Download the app onto your device, then open it.
  4. Sign in using your normal Adit App login and password.

  5. Select your location from the dropdown. If your organization only has one location, it will be selected by default.
  6. Give this device a nickname in the Device Name field. This is especially important if you will have multiple devices connected, as it will allow you to tell them apart.
Your device is now connected to your Adit account and you can push patient forms to it as needed.

To remove a device from your Adit account, navigate to the Patient Forms module, then Preferences > Forms. You will see a list of connected devices; click Disconnect beside the device you would like to remove.
