How To: View and modify a patient's form history
When you assign a form through Adit's Patient Forms module, we automatically track which form was assigned and when. When the patient submits a form through Adit's Patient Forms, we log that information as well. You can view all of this data for each patient in their Form History.
Adit uses the Form History to automate assigning forms to patients on a recurring basis. This is useful for things like asking patients to update their medical history every 12 months.
To view a patient's Form History, open their Patient Card and click on the Patient Forms tile.
The top section shows forms that are currently assigned to the patient but still pending. You can archive these requests by clicking the Archive icon
beside the form name. If you would like to assign an additional form to this patient, click the
Assign Form button.
The bottom section of the form history shows all of the forms that this patient has submitted through Adit's Patient Forms feature and the date of each submission. You also have the option to manually log a form by clicking the Add Completed Form button. This allows you to log the date that a form was submitted outside of Adit's Patient Forms system (such as a paper copy) and ensures the Form History remains intact to accurately assign recurring forms.
