Many practices like to attach consent forms or informational handouts to a treatment plan for the patient to sign off on.

Note: In your Adit platform, you will need to add the consent form or handout to your
Patient Forms module before you can attach it to a Treatment Plan. For help adding a form to your Patients Form module, see our article on how to
Create a Patient Form.
To attach a patient form or consent form to a treatment plan:
- Create the Treatment Plan. For more detailed instructions, see how to Create a Treatment Plan.
- Scroll down to the Consent Forms section at the bottom of the plan builder.
- Click the button to Attach Consent Form from Adit’s Patient Forms.
- In the pop-up box, click the drop-down menu to see a list of your active forms from your Patient Forms module.
- Check the box beside each form you would like to attach to this Treatment Plan. You can select up to 5 forms.
- Click the Done button.
- Finish building the Treatment Plan and send it to your patient.

Note: If you have already sent a Treatment Plan to a patient, you can edit the plan to attach forms up until the patient submits the Treatment Plan.
When the patient receives the Treatment Plan, they will not be able to complete or accept the plan until they complete all the attached Patient Forms. To complete a form, click on the Start or Complete button beside each attached form and follow the provided instructions. The page will redirect back to the Treatment Plan, where the patient can then click on the next form and repeat until they have completed all of the attached Patient Forms. Only when all attached Patient Forms are completed will the Submit button at the bottom of the Treatment Plan be available to click, which completes their Treatment Plan.
