View a Patient Form

The Patient Forms dashboard allows you to see forms that have been requested, submitted, and approved, each in an easy, filterable list.

 

To view a patient form:

  1. Navigate to the Patient Forms module. Patient Forms icon
  2. In the top navigation menu, click on Submission.  If you don’t see this option in the menu, click the three dots to view the rest of the menu options.
  3. Choose which list view you’d like to access:
    Patient Forms Page Header
    1. All - shows all form requests and submissions
    2. Requested - shows forms that your staff requested from patients, but are incomplete. This will also show forms which were sent to patients via Auto-Assign.
    3. Submitted - shows forms that patients submitted, regardless of completion.
    4. Imported - shows forms that your staff approved and pushed to your EHR.
    5. Archived - shows forms that your staff archived from your Patient Forms dashboard.
  4. To search for a particular patient by name, click into the Search field near the top right corner of the form list.
  5. To filter for a particular date range, click the Date Range field near the top right corner of the form list and choose your desired range, then click Apply.
  6. Click the patient’s name to open and view their form.
  7. From this screen, you can also Download a PDF of a form, Approve a form, and Archive a form.

If your EHR software is integrated with the Adit platform, Approved forms are also imported into your patient record automatically and can be viewed in the EHR as well.

To approve a patient form:
  1. Navigate to the Patient Forms module. Patient Forms icon
  2. In the top navigation menu, click on Submission.  If you don’t see this option in the menu, click the three dots to view the rest of the menu options.
  3. Click the Submitted tab.
  4. Locate the form you want to approve.
  5. Click the Approve button.
  6. Select existing patient by typing in their name or create a new patient profile.
  7. Click Approve.
