Condense or Expand Patient Form Layout
Adit’s Patient Forms offer three PDF layouts for your forms: Compressed, Table Layout, and Large Print Layout. Changing the PDF layout does not change how the form appears on the screen when a patient is filling it out, this only modifies how the PDF appears when it is downloaded.
To condense or expand the PDF form layout:
Navigate to the Patient Forms module.
In the top navigation menu, click on Form Builder.
Select the form you want to customize the PDF layout for.
In the form builder, select the Download dropdown and, choose which layout you prefer:
Compressed Layout reduces the size of the file, removing any unused/white space.
Table Layout puts each field/questions in its own cell in the PDF.
Large Print Layout is the way the form appears online.
