Customize Patient Form PDF Settings

When downloading forms, you can set the form template to be automatically saved with a specific name format and PDF layout. You can also set the folder destination when the form is imported into your patient management system.


To customize these settings:

  1. Navigate to the Patient Forms module.

  2. In the top navigation menu, click on Form Builder.

  3. Select the form you want to edit

  4. In the form builder, you have the following options when customizing your file:

    1. Folder

      1. You can choose which folder this form will upload to when it is imported into the EHR.

    2. Form Name

      1. You can choose the format of how the name of the form will be displayed when it is either downloaded as a PDF or imported into the EHR.

      2. The  formatting options available are variations of the following:

        1. Form Name

        2. Date Submitted

        3. Patient Name

    3. Download

      1. You can choose the format of how the form layout will look when it is downloaded as a PDF.
