Assign Forms From the Patient Card
The patient card makes it easy to manage a patient’s details all in one place.
Here is how to assign a form from the patient card:
To open up a patient’s card, type your patient’s name into the Search box, which you can find at the top of any page within the Adit platform.
Select the patient from the list of suggested matches, and their Patient Card will open on the right-hand side of your screen.
Under activity, the first section will be forms.
These are the different scenarios you can come across:
When patient forms is not enabled
When there aren’t any forms assigned to the patient yet
When there are forms assigned to the patient
You can assign a form by clicking on the Assign Form button.
From the pop up, you can select the forms you want to assign to the patient. Please note that you can only select a maximum of 5 at a time.
Click on Assign
Confirm the message that will be sent to the patient
Click on Send
The patient will receive an email or text message with a link to fill out their patient forms.
