How To: Set up Pre-Fill Fields in Patient Forms

Adit’s Patient Forms allow practices to pre-fill certain form fields before sending a form to a patient.

There are two types of fields that can be pre-filled: single line text fields and date fields. 

To add a Pre-Fill Field to a patient form:
  1. Navigate to the Form Manager and edit the desired form.
  2. Select the field you would like to set as a Pre-Fill Field. You can edit an existing field, or add a new field to the form.
  3. In the right-hand panel, toggle the Pre-Fill Field option ON.
  4. Save your changes.
Pre-Fill Fields cannot be required fields.
Please note that pre-fill fields cannot be edited by the patient. If a pre-fill field is left empty, it will remain empty even after the patient fills out and submits the form.
