How to Manually Import Forms into a Patient Profile

In some cases, a form may be submitted without being tied to a specific patient profile. When this happens, the practice will need to manually import the form to the correct patient’s file. This article outlines the steps for locating unassigned forms, approving them, and linking them to the appropriate patient record.

When Manual Import is Required

This process is necessary when:

  • A form is sent manually without pre-assigning to a patient profile.

  • A kiosk or shared device is used, and no patient is pre-selected.

  • You have configured your preferences in the forms manager to manually import the forms vs. automatically.

Step 1: Notification of Submission

When a form is submitted without being assigned to a patient, a notification will appear in:

  • The Notifications Center within the Adit dashboard

  • Email alerts (if enabled)

  • The alert section of the Patient Forms module

This alert indicates that manual action is needed to process the form.

Step 2: Access the Patient Forms Module

  1. Navigate to the Patient Forms module in your Adit dashboard.

  2. Locate the form submission marked with an orange "Approve" button.

Step 3: Approve the Form

  1. Click the "Approve" button next to the unassigned form.

  2. A pop-up window will appear with two options:

    • Link to an existing patient profile

    • Create a new patient profile

Option A: Link to an Existing Patient
Select "Link to an existing patient profile." Use the search bar to locate the correct patient by name, phone number, or email. Select the appropriate patient and confirm the link.

Option B: Create a New Patient
Select "Create a new patient profile." Enter the necessary information, such as the patient’s name, date of birth, and contact details. Save the profile to link the form.

Step 5: Form is Imported

Once the form is linked, it will be imported into the designated patient's file. It will be accessible from:

  • The Documents tab within the patient's profile.

The form will now be part of the patient’s record and can be used for review, compliance, and documentation purposes.

Best Practices

  • Review the Patient Forms module regularly to stay on top of unassigned submissions.

  • Use accurate patient details to avoid duplicate profiles.
