Adit Forms Library contains a robust and diverse set of templates for your practice to utilize in creating and sending patient forms. 

1. Navigate to the Patient Forms module, then select Form Builder

2. Select Library

3. From here, you can view a form by selecting Preview or selecting Use Template to bring up the form editor

      a. Selecting Preview will show what the form will look like to a patient

      b. Selecting Use Template will bring up the form editor and will allow you to make changes or save the form as is if desired.
