Patient medications can easily be pulled from your EHR and be placed into Adit’s Patient Forms. When patients select a medication in their form, that data will then be imported back into their patient chart.
To add medications to a patient form:
Navigate to the Patient Forms module.
In the top navigation menu, click Form Manager . If you don’t see this option in the menu, click the three dots to view the rest of the menu options.
Click the Create Form button to create a new form, or click Form Name, or Edit icon on an existing form you want to update.
On the Form building page, you will see a tool bar on the left side. This tool bar provides different types of fields that you can use in your form.
Go to the custom fields section and drag the medications field into your form.
You’ll see a pop-up appear, explaining that this field will automatically populate a list of medications based on the master list in your EHR. Select Yes to continue.
Once the field is placed on your form, the right hand column will appear showing the field properties.
Field Label - Use this field to write the question you want to ask the patient
For Example: “Are you currently taking any medications?”
Hidden - you can hide this question on the patient form
Required - you can make this field mandatory for patients to fill out
Copy - this option is disabled for all custom fields because you can only have one in each form
PMS Field - this field allows you to map where the data should write into your EHR. Since this is a custom field, it will be automatically mapped to the medications in the patient’s chart.
Medication Name - these are automatically pulled based on the medications available in your EHR.
You can choose to select the Eye Iconto hide a medication from the dropdown or select the X to remove it completely.
Please note if you want to add any medications on this list, it will have to be added directly in your EHR first, and then once that information syncs over you have to remove and re-add this medication field in your form.
Reset - If you want to remove any updates you made to the medications field, you can select this buttonto start over.
Once you are satisfied with your form and are ready to start using it, go to the top navigation and select either Save or Save & Exit
Now once you send a form request to a patient, this is how the medications field will look like from a patient’s perspective.
Patients who have medications associated to their profile in the EHR will have the medications automatically pre-populated when they receive a form request.
Patients will also be able to enter in new medications by using either of these methods:
Clicking on the dropdown and selecting from a list of predefined medications
Note: medications selected from the dropdown will be imported over to the patient’s profile once the form is approved
Manually entering in their own medication
Note: medications manually entered in will not be imported over.
Patients can add more medications by clicking on + or remove a medication by clicking on X
